Case Management is framed around identifying an individual’s medical, social, and functional needs, then working directly through community resources to address these needs while the substance use disorder is being treated. Common areas case managers assist individuals with: obtaining identification records (birth certificate, social security card, state i.d.), housing, employment, school/education, medical/dental appointments, transportation, food resources, and more.
